- Assess, evaluate, and plan patient care in accordance with Physical Therapy standards, focusing on adult and geriatric populations.
- Provide skilled physical therapy treatments and interventions tailored to individual patient diagnoses, including orthopedic, neurological, and mobility conditions.
- Document patient progress, treatment plans, and outcomes accurately using NetSmart EMR, ensuring compliance with regulatory and reimbursement standards.
- Collaborate with interdisciplinary team members and supervisors to ensure comprehensive patient care and effective communication.
- Manage and prioritize a caseload aiming for an average of 4.5 visits per day while maintaining quality and efficiency.
