This position involves evaluating, planning, and implementing individualized treatment plans to improve mobility, strength, and function for patients recovering from injuries, surgeries, or managing chronic conditions.
Key Responsibilities:
- Conduct initial and ongoing evaluations of patients' physical conditions in their home setting.
- Develop and implement individualized treatment plans based on patient goals and medical needs.
- Provide therapeutic interventions to improve strength, balance, coordination, and mobility.
- Educate patients and caregivers on techniques and exercises to support recovery and wellness.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other therapists, to ensure comprehensive patient care.
- Document progress, outcomes, and modifications to treatment plans accurately and in a timely manner.
- Comply with all state and federal regulations, including HIPAA, and company policies and procedures.
Qualifications:
- Licensed Physical Therapist (PT) in the state of Florida.
- Experience in home health or outpatient therapy settings preferred.
- Strong knowledge of physical therapy techniques, principles, and modalities.
- Excellent communication and interpersonal skills to work with patients, families, and the healthcare team.
- Ability to work independently and manage time effectively.
- Reliable transportation for travel within Brevard County.
