We are seeking a compassionate and self-motivated Per Diem Physical Therapist (PT) to join our growing home health team in Austin, Texas. This flexible, patient-centered role is ideal for a licensed Physical Therapist who thrives in a one-on-one care setting and enjoys autonomy in their daily schedule.

Whether you're looking to supplement your hours or build a career in home health, this is a rewarding opportunity to make a meaningful impact on patients in the comfort of their homes. At least 10 appointments per week are available to pick up, but there is only a minimum of 2 appointments / week required.

Job Responsibilities

  • Conduct in-home assessments and develop individualized care plans

  • Provide hands-on physical therapy services to patients recovering from illness, injury, or surgery

  • Set functional and measurable therapy goals in collaboration with patients and their families

  • Monitor patient progress and update care plans accordingly

  • Educate patients and caregivers on exercises, safety, and recovery techniques

  • EMR System: KanTime – Training provided if needed

  • Communicate effectively with case managers, physicians, and other care team members

Qualifications

  • Licensed Physical Therapist (PT) in the state of Texas

  • Home health experience strongly preferred

  • Excellent interpersonal and organizational skills

  • Ability to work independently and manage a flexible caseload

  • Reliable transportation and willingness to travel to patient homes within the Austin area
