We’re hiring a licensed and dedicated Per Diem Physical Therapist (PT) to join our expanding home health team in Austin, Texas. This is a highly flexible, field-based position perfect for PTs who thrive in providing one-on-one care and appreciate the freedom of setting their own schedule.

Whether you’re seeking to supplement existing hours or deepen your experience in the home health space, this opportunity offers the chance to build meaningful patient relationships. A minimum of 5 weekly visits is required. If desired, we can guarantee up to 32 visits weekly.

Key Responsibilities

  • Perform comprehensive in-home evaluations and create tailored treatment plans

  • Deliver direct physical therapy services for patients recovering from surgeries, injuries, or health setbacks

  • Establish measurable goals in collaboration with each patient and their family

  • Track and assess progress, making updates to care plans as needed

  • Instruct patients and caregivers on rehab exercises, safety techniques, and self-care

  • Use KanTime as the documentation system – training available if needed

  • Collaborate with physicians, case managers, and other clinicians to ensure cohesive care

Qualifications

  • Current Physical Therapist (PT) license in Texas

  • Prior home health experience is highly preferred

  • Strong organizational and interpersonal skills

  • Ability to manage a mobile caseload and work independently

  • Reliable vehicle and valid driver’s license to travel locally in the Austin area
