Job Summary:
We are seeking a compassionate and skilled Physical Therapist to join our Home Health team on a per diem basis. As a Physical Therapist, you will play a vital role in providing high-quality patient care in the comfort of their own homes. This is an excellent opportunity for a licensed therapist looking for flexibility and a rewarding experience.
Key Responsibilities:
- Evaluate patients' physical abilities and limitations to develop personalized treatment plans
- Provide skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, and overall well-being
- Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal patient outcomes
- Conduct assessments, implement treatment plans, and document patient progress
- Work independently and make sound judgments regarding patient care and treatment plans
Requirements:
- Current Texas Physical Therapy License (TX PT State License)
- Preferred 1 year of experience in Home Health, but not required (training is offered)
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
