- Provide high-quality home health physical therapy services to geriatric patients in rural settings within assigned counties
- Conduct evaluations, develop, and implement individualized treatment plans tailored to patient needs
- Perform patient visits, admissions, and discharges efficiently while maintaining documentation accuracy in EMR system (HCHB)
- Collaborate with the interdisciplinary team to ensure comprehensive patient care and progress monitoring
- Adhere to scheduling and coverage requirements, including floating within designated counties and following facility protocols
