Job Requirements
  • Education Requirements: Bachelor's degree in Physical Therapy or related field
  • Certifications Requirements: Current state-issued Physical Therapist license; CPR certification; Malpractice insurance
  • Experience Requirements: At least 1 year of home health experience; Experience with EMR systems (Matrix preferred); Proven ability to manage a caseload efficiently
  • Other Requirements: Ability to travel within a 30-mile radius; Strong communication and interpersonal skills; Ability to work independently in a home health setting

Job Responsibilities
  • Provide comprehensive physical therapy assessments and develop personalized treatment plans for home health patients
  • Execute therapeutic interventions to improve patient mobility, strength, and overall functional abilities
  • Maintain accurate and timely documentation of patient progress in the EMR system (Matrix)
  • Collaborate with interdisciplinary teams, patients, and families to ensure optimal care delivery
  • Manage a caseload of 5-6 patient visits per day within a 30-mile radius, ensuring adherence to schedule and treatment goals
