- Certifications Requirements: Current state-issued Physical Therapist license; CPR certification; Malpractice insurance
- Experience Requirements: At least 1 year of home health experience; Experience with EMR systems (Matrix preferred); Proven ability to manage a caseload efficiently
- Other Requirements: Ability to travel within a 30-mile radius; Strong communication and interpersonal skills; Ability to work independently in a home health setting

- Execute therapeutic interventions to improve patient mobility, strength, and overall functional abilities

- Collaborate with interdisciplinary teams, patients, and families to ensure optimal care delivery
- Manage a caseload of 5-6 patient visits per day within a 30-mile radius, ensuring adherence to schedule and treatment goals
