Connected Health Care is partnering with a highly regarded school district in Wayne Township, New Jersey, to hire a School Social Worker – Case Manager for an immediate, full-time contract role. This position plays a vital role in supporting students' emotional and behavioral needs, while collaborating with the Child Study Team (CST) to ensure comprehensive case management and advocacy.

Key Responsibilities

Conduct assessments to identify students' emotional, behavioral, and social needs
-
Manage assigned caseloads and ensure compliance with timelines and special education regulations
-
Provide individual and group counseling to students facing social-emotional challenges
-
Support the development and implementation of IEPs, 504 Plans, and Behavior Intervention Plans (BIPs)
-
Collaborate with school staff and external agencies to coordinate mental health services
-
Participate in CST meetings, parent consultations, and intervention planning
-
Advocate for students and promote a safe, inclusive, and positive school culture
-
Support students experiencing trauma, homelessness, or crisis situations
Requirements

New Jersey School Social Worker Certification (Required)

Master’s Degree in Social Work (MSW) or related field (Required)

Experience working with students in school settings

Knowledge of behavioral support strategies, SEL programs, and crisis intervention

Ability to collaborate within a multidisciplinary CST team
