As a Social Worker, you will play a vital role in coordinating and providing comprehensive psychosocial care to patients. Located in the picturesque Mogollon Rim region of Arizona, Payson offers an abundance of natural beauty and recreational opportunities. Surrounded by the Tonto National Forest, you will have easy access to hiking, fishing, camping, and breathtaking views of the rugged landscapes.

Your primary Social Worker responsibilities will include:

• Conducting psychosocial assessments to determine patients' needs, preferences, and goals
• Collaborating with the interdisciplinary team to develop individualized care plans
• Providing counseling and support to patients and their families
• Collaborating with external agencies and organizations to coordinate post-hospital care and services

To be successful in this role, you should possess a Master’s Degree in social work from an accredited school (preferred). We are also looking for candidates with:

• Basic computer skills (required)
• BLS/CPR certification (Preferred upon hire; required within 30 days of hire and must be maintained throughout employment)
• AZ fingerprint clearance card (Application completed within 14 days of hiring date; required)
• One year related experience in crisis intervention and/or hospital social work, preferred
• Licensure (LMSW) in Social Work within six months of hire (required)
