Location: Warren, MA
Pay Rate: Up to $55/hr | Schedule: Full-Time | Onsite
Caseload: Approximately 8 students in a self-contained classroom with inclusion opportunities
Assignment Duration: ASAP through June 16, 2026
Position Overview:
Connected Health Care is seeking a passionate and experienced Special Education Teacher (Moderate/Severe) to join an elementary school team in Warren, MA. This full-time role focuses on providing intensive academic, behavioral, and life skills instruction for a small group of K–3 students with moderate to severe disabilities.
Key Responsibilities:

Deliver individualized instruction in a self-contained moderate/severe program with some inclusion support

Implement and monitor IEPs, behavioral plans, and communication systems
-
Support students in developing academic, functional, and social-emotional skills
-
Provide structure, positive reinforcement, and differentiated instruction
-
Collaborate with related service providers, paraprofessionals, and families
