Are you a passionate Speech Language Pathologist (SLP) seeking a fulfilling opportunity? Connected Health Care, a top recruitment and staffing firm, is currently seeking an experienced SLP to join our team for a 13-week contract in a Skilled Nursing Facility. If you are committed to helping patients improve their communication and swallowing abilities, this role is perfect for you!

Position Summary: As a Speech Language Pathologist, you will provide direct rehabilitation care in speech therapy to patients referred by a physician, ensuring adherence to all applicable laws, regulations, and standards. You will be responsible for developing, assessing, and modifying individualized treatment plans to achieve the best possible outcomes for patients.

Key Responsibilities:

  • Deliver exceptional speech therapy services in a Skilled Nursing Facility setting.
  • Establish, assess, and adjust treatment plans with realistic, measurable, and functional goals.
  • If ASHA-certified (CCC), oversee and evaluate care provided by SLP Clinical Fellows (CFYs).
  • If a CFY, follow treatment plans under the supervision of an ASHA-certified SLP.
  • Maintain accurate and timely patient records using therapy software.
  • Collaborate effectively with interdisciplinary teams to ensure comprehensive patient care.
  • Uphold professional standards and confidentiality in all interactions.

Requirements:

  • Graduate of an accredited Speech Therapy program (minimum MS or MA in SLP).
  • ASHA certification (CCC) or eligibility for a Clinical Fellowship Year (CFY).
  • Active Speech Language Pathology license in the applicable state.
  • CPR certification (required upon hire or during orientation).
  • Preferably one year of experience in post-acute care or a related setting.
  • Proficient in Microsoft Word, Excel, and email communication.
  • Strong interpersonal skills and the ability to work both independently and as part of a team.
