Description: Connected Health Care, in partnership with our client, is seeking a skilled Speech-Language Pathologist (SLP) to join a reputable Skilled Nursing Facility (SNF) in the picturesque area of Albany/St. Cloud/Long Prairie, MN. As an SLP, you will be responsible for providing high-quality speech therapy services to patients within a skilled nursing environment. Responsibilities:

• Conducting patient assessments and developing individualized treatment plans
• Providing therapy services to patients with speech, language, cognitive, and swallowing disorders
• Collaborating with interdisciplinary team members to optimize patient care
• Documenting patient progress and maintaining accurate records
• Providing patient and family education on speech therapy techniques and strategies

Qualifications:

• Master’s degree in Speech-Language Pathology
• Valid SLP license in the state of Minnesota
• Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA)
• Experience working in a skilled nursing facility preferred, but new graduates are welcome to apply
