Job Requirements
  • Education Requirements: Valid Registered Nurse (RN) license in Illinois.
  • Certifications Requirements: BLS (Basic Life Support); ACLS (Advanced Cardiovascular Life Support) preferred but not mandatory; Successful completion of an accredited nursing program.
  • Experience Requirements: Minimum of 1 year of clinical nursing experience in long-term care or rehabilitation settings.

Job Responsibilities
  • Provide professional nursing care to residents in long-term care and rehabilitation facilities, ensuring adherence to all clinical guidelines and safety protocols.
  • Assess residents' health status, develop individualized care plans, and modify treatments as necessary in collaboration with healthcare teams.
  • Document all patient interactions, treatments, and observations accurately in the Electronic Medical Record (EMR) system.
  • Administer medications and treatments in accordance with facility policies and state regulations.
  • Communicate effectively with residents, families, and interdisciplinary team members to promote optimal patient outcomes.
