Travel Case Manager RN (Hospice) – Spokane, WA
Contract Length: 12 Weeks | Weekly Compensation: Up to $2,600 DOE
Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Join a supportive team dedicated to delivering exceptional end-of-life care to patients and their families in a breathtaking location.
Job Description:

Coordinate and manage individualized care plans for hospice patients

Perform in-home assessments and ongoing patient evaluations
-
Collaborate with interdisciplinary teams including physicians, social workers, and chaplains
-
Provide patient and family education on disease progression and care expectations
-
Maintain accurate documentation using Matrix/Bright Tree systems
-
Ensure compliance with federal, state, and agency regulations
-
Deliver compassionate nursing care that prioritizes quality of life and dignity
Education & Certification Requirements:
-
Active RN license in the state of Idaho or compact license
-
Minimum of 1 year of hospice case management experience preferred

Current BLS (Basic Life Support) certification
-
Strong knowledge of hospice philosophy and best practices
-
Proficiency with EMR systems, ideally Matrix/Bright Tree
