Travel Case Manager RN (Hospice) – Spokane, WA
Contract Length: 12 Weeks | Weekly Compensation: Up to $2,600 DOE
Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Connected Health Care, a leading recruitment and staffing firm, is seeking a compassionate and experienced Travel Hospice Case Manager RN for a rewarding opportunity in Spokane, WA. Join a supportive team dedicated to delivering exceptional end-of-life care to patients and their families in a breathtaking location.

Job Description:

  • Coordinate and manage individualized care plans for hospice patients

  • Perform in-home assessments and ongoing patient evaluations

  • Collaborate with interdisciplinary teams including physicians, social workers, and chaplains

  • Provide patient and family education on disease progression and care expectations

  • Maintain accurate documentation using Matrix/Bright Tree systems

  • Ensure compliance with federal, state, and agency regulations

  • Deliver compassionate nursing care that prioritizes quality of life and dignity

Education & Certification Requirements:

  • Active RN license in the state of Idaho or compact license

  • Minimum of 1 year of hospice case management experience preferred

  • Current BLS (Basic Life Support) certification

  • Strong knowledge of hospice philosophy and best practices

  • Proficiency with EMR systems, ideally Matrix/Bright Tree
