Travel Case Manager RN – Medford, WI
13-Week Contract | Weekly Pay Up to $2,700 (DOE)

Connected Health Care, a leading recruitment and staffing firm, is seeking an experienced Travel/Contract Case Manager Registered Nurse for a highly respected home health facility in Medford, Wisconsin. If you're a compassionate, organized RN with a strong background in home health, this is an excellent opportunity to bring your skills to a supportive environment while exploring the scenic beauty of north-central Wisconsin.

Job Description:

  • Provide comprehensive case management to patients in a home health setting

  • Perform assessments, create care plans, and coordinate multidisciplinary services

  • Utilize EPIC and OASIS systems for accurate documentation and compliance

  • Monitor patient progress, revise care plans as needed, and ensure timely communication with healthcare providers and families

  • Educate patients and caregivers on disease processes, medications, and self-care techniques

  • Maintain travel flexibility with potential floating to other territories during low census

  • Work collaboratively with interdisciplinary teams to support holistic patient care

Shift & Schedule:

  • 8:00 AM – 4:30 PM, 8-hour day shifts

  • Every other weekend required

  • Holiday rotation

  • Travel to patient homes required – valid driver’s license necessary

Qualifications:

  • Active Wisconsin RN license

  • AH ABLS and valid driver’s license required

  • OASIS certification is a plus

  • Minimum 2 years of home health experience strongly preferred

  • Previous travel RN experience required

  • Proficient with EPIC and OASIS charting systems strongly preferred

  • Strong critical thinking, time management, and communication skills
