Travel Home Health Registered Nurse - Portage, WI

Connected Health Care is seeking an experienced and self-motivated Travel Home Health Registered Nurse for a 13-week assignment in Portage, WI. If you’re passionate about providing personalized care in the comfort of patients’ homes and have experience with OASIS documentation, this is a perfect opportunity to make a meaningful impact while enjoying the beautiful Midwest.

Assignment Details:

  • Contract Length: 13 Weeks

  • Shift: 8:00 AM – 4:30 PM | 8-Hour Days

  • Weekly Compensation: Up to $2,700/week (based on experience)

Job Description:

  • Provide in-home nursing care to a diverse population of patients

  • Complete head-to-toe assessments and deliver evidence-based interventions

  • Document patient visits using OASIS (required) and Epic (preferred) charting systems

  • Educate patients and families on disease management, medications, and care plans

  • Coordinate with physicians, case managers, and other healthcare professionals

  • Ensure compliance with all regulatory requirements and safety protocols

  • Travel locally throughout the service area using personal vehicle

Education & Certification Requirements:

  • Active Registered Nurse license in Wisconsin or Compact State license

  • AHA BLS certification required

  • Valid driver’s license and reliable transportation required

  • OASIS certification a PLUS

  • Minimum of 2+ years of Home Health experience preferred

  • Travel nursing experience required
