Shift: 8-Hour Days | 8:00 AM - 4:30 PM (Flexible Start/End Times)
Contract: 13 Weeks
Guaranteed Hours: 40 per week

We are currently seeking an experienced Home Health Registered Nurse (RN) for a travel assignment in Rhinelander, WI. Job Responsibilities

• Provide direct patient care in a home health setting, ensuring individualized and comprehensive treatment plans
• Conduct OASIS assessments and maintain accurate documentation in EPIC
• Administer medications, IV therapy, wound care, lab draws, and chronic disease management
• Collaborate with physicians and healthcare teams to coordinate optimal patient care
• Triage patient calls and determine appropriate interventions, including in-home visits when necessary
• Travel within the assigned territory, averaging 4.5 visits per day (approx. 50-100 miles per day)
• Work every other weekend and rotate holidays as part of the care team
• Participate in on-call duties (3-5 days per month + potential weekends) with a 15-minute response time for triage calls
• Potential to float to other territories as needed during low census periods

Education and Certification Requirements

• Active RN license in Wisconsin (or Compact State)
• BLS (AHA or ARC) certification required
• Valid Driver’s License (required for travel between patient homes)
• OASIS Certification preferred (willing to train)
• Minimum 1 year of Home Health experience required (2+ years strongly preferred)
• Travel nursing experience preferred

Skills & Competencies

• Proficiency in OASIS documentation and EPIC charting (preferred)
• Strong skills in wound care, IV therapy, ports, and lab draws
• Excellent time management, adaptability, and communication skills
