Job Requirements
- Education Requirements: Graduation from an accredited nursing program with an active RN license in Wisconsin or a valid compact state license
- Certifications Requirements: AHA BLS certification, OASIS certification (a plus), Valid Driver's license
- Experience Requirements: Minimum of 2+ years of home health nursing experience, prior travel experience, proficiency with Oasis documentation, and experience with EMR systems like Epic
- Other Requirements: Strong time management skills, resilience, dependability, compassionate nature, positive attitude, and ability to manage change effectively

Job Responsibilities
- Provide comprehensive home health nursing care to patients in their homes, following physician orders and care plans
- Assess patient needs, develop individualized care plans, and execute nursing interventions accordingly
- Document patient care activities accurately and timely in EMR systems such as Oasis and Epic
- Coordinate with physicians, therapists, and other healthcare team members to ensure continuity of care
- Educate patients and families on disease management, medication administration, and health promotion strategies
