Job Description:
- Provide comprehensive hospice care to patients in their homes or healthcare facilities.
- Assess patient needs, develop individualized care plans, and adjust treatment as needed.
- Administer medications, pain management, and symptom control treatments.
- Offer emotional support and education to patients and their families.
- Collaborate with physicians, social workers, and hospice care teams to ensure patient comfort.
- Maintain accurate documentation of patient progress and care plans.
- Ensure compliance with hospice regulations, policies, and ethical standards.
Education & Certification Requirements:
- Active RN license in Wisconsin (or compact state license).
- Bachelor of Science in Nursing (BSN)Â preferred, but Associate Degree in Nursing (ADN) accepted.
- Basic Life Support (BLS) certification required.
- Hospice and Palliative Care Nursing Certification (CHPN)Â preferred.
- Minimum 1 year of hospice or palliative care nursing experience required.
