Licensed Practical Nurse (LPN) – Long Term Care | 13-Week Contract
Facility: Edgewater Haven
Location: Port Edwards, WI
Shift: 1:45 PM – 10:15 PM (8-Hour Variable Shifts)
Contract Length: 13 Weeks

About the Opportunity:

Connected Health Care is seeking a compassionate and experienced Long-Term Care Licensed Practical Nurse (LPN) for a travel/contract assignment at Edgewater Haven in Port Edwards, WI. Job Details:

  • Guaranteed Weekly Hours: 32 hours (potential for up to 40 based on census needs)
  • Shift Details:
  • 1:45 PM – 10:15 PM (MIDS)
  • Every other weekend: 6:00 PM – 6:00 AM
  • Fill in on weekdays to meet contracted requirements
  • Floating: All three wings of the facility
  • Holidays: Every other holiday rotation

    Unit Information:

  • 300 Wing: 33 beds (current census: 28), mix of hospice, LTC, and rehab patients.
  • 500 Wing: Alarmed memory care unit with dementia patients (25 beds, current census: 16).
  • Facility licensed for 79 beds (all private); typical census is 45-52 patients.

    Qualifications:

  • License/Certifications: Wisconsin LPN and AHA BLS required.
  • Experience: Minimum 1-2 years in a long-term care or similar setting.
  • Skills: Hard-working, resilient, adaptable to change, and socially engaging.
  • Covid-19 Vaccine: Required.

    Dress Code:

  • Scrubs (any color).
  • Jeans allowed on Mondays and Fridays; khakis or appropriate-length shorts acceptable at other times.
  • Closed-toe shoes required.
  • Facial piercings must not be of the hoop variety.
