Job Title: Local Travel LVN – Chester, CA (LTC & Acute Care)

Job Description: Connected Health Care is partnering with a respected healthcare provider in Chester, CA, to find a Local Travel Licensed Vocational Nurse (LVN) for a 13-week contract assignment. The ideal candidate will have experience in both Long-Term Care (LTC) and Acute Care settings. Key Responsibilities:

• Provide direct patient care in both LTC and Acute Care settings.
• Administer medications, treatments, and assist in patient assessments.
• Monitor patients' conditions and report changes to the healthcare team.
• Perform routine and emergency nursing duties under the supervision of registered nurses and physicians.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Educate patients and their families on managing medical conditions, medications, and lifestyle changes.
• Ensure accurate and timely documentation of patient care activities in compliance with healthcare regulations.
• Maintain a clean, safe, and supportive environment for patients and families.
• Participate in patient care meetings and quality improvement initiatives as required.

Education & Certification Requirements:

• Graduation from an accredited LVN program.
• Current LVN licensure in California.
• BLS (Basic Life Support) certification required.
• Experience in both LTC and Acute Care settings preferred.
• Strong communication and interpersonal skills, with the ability to work effectively as part of a team.
• Ability to handle various patient needs with compassion and professionalism.
• Must be flexible and adaptable in a dynamic healthcare environment.

Contract Details:

• Contract Length: 13 weeks.
• Weekly Compensation: Up to $2,268 based on experience.
