Job Title: Local Travel LVN – Near Susanville, CA (LTC & Acute Care)

Job Description: Connected Health Care is currently working with a healthcare provider near Susanville, CA, to find a dedicated and skilled Local Travel Licensed Vocational Nurse (LVN) for a 13-week contract assignment. The ideal candidate will have experience in both Long-Term Care (LTC) and Acute Care settings. Key Responsibilities:

• Provide patient care in both LTC and Acute Care settings, ensuring a high level of quality care and compassion.
• Administer medications, perform treatments, and assist in the assessment and care of patients.
• Monitor patients’ conditions and report any changes or concerns to the healthcare team.
• Participate in patient care planning and collaboration with other medical professionals to ensure optimal care.
• Educate patients and their families on managing chronic conditions, medications, and improving overall well-being.
• Ensure accurate documentation of all patient care and interactions in accordance with regulations.
• Assist in maintaining a clean, safe, and supportive environment for patients.
• Respond to emergencies as needed, providing basic life support and urgent care under the supervision of RNs and physicians.
• Adhere to hospital and state protocols for infection control, patient care, and safety.

Education & Certification Requirements:

• Graduation from an accredited LVN program.
• Active LVN licensure in California.
• Current BLS (Basic Life Support) certification.
• Previous experience in both LTC and Acute Care settings preferred.
• Strong communication skills and the ability to work well within a healthcare team.
• Compassionate and professional approach to patient care, with the ability to handle diverse patient needs.
• Ability to adapt and be flexible in a dynamic work environment.

Contract Details:

• Contract Length: 13 weeks.
• Weekly Compensation: Up to $2,268 based on experience.
• Local travel position with competitive compensation.
