Connected Health Care is seeking a passionate and skilled Travel Physical Therapist for a 13-week contract assignment with one of our trusted healthcare partners in Hardeeville, SC. Job Description:

Evaluate patients to develop individualized treatment plans to improve mobility, relieve pain, and prevent or limit physical disabilities.

Implement physical therapy interventions based on clinical judgments and patient needs.

Collaborate with physicians, nurses, and other healthcare team members to provide coordinated care.

Document patient progress, treatment, and outcomes accurately and timely.

Educate patients and family members on treatment plans, exercises, and recovery expectations.

Maintain compliance with state and federal guidelines and facility protocols.
Education & Certification Requirements:

Degree in Physical Therapy from an accredited program (Bachelor’s, Master’s, or Doctorate)

Active South Carolina Physical Therapy License (or eligibility to obtain)

Current BLS (Basic Life Support) Certification

Prior experience in a travel or outpatient/inpatient setting preferred, but not required
