- Education Requirements: Valid Physical Therapist license in the state of Oregon
- Certifications Requirements: CPR certification, BLS certification, and applicable state licensure
- Experience Requirements: Minimum of 1 year of outpatient physical therapy experience
- Other Requirements: Ability to work 8 am - 5 pm shifts, excellent communication skills, reliable transportation, flexibility to float to other clinics, and proficiency in electronic medical records (EMR) systems
