Job Description:

• Provide physical therapy services to patients in a home health setting.
• Develop and implement personalized treatment plans based on patient assessments.
• Conduct patient evaluations and document progress accurately.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Educate patients and caregivers on therapeutic exercises and home care techniques.

Education and Certification Requirements:

• Valid Physical Therapy license in New Mexico.
• Minimum of 2 years of experience, with home health experience preferred.

Contract Details:

• Contract Length: 13 weeks
• Weekly Compensation: Up to $2,600 based on experience
