Job Description:
Connected Health Care is seeking a dedicated and skilled Local Travel Physical Therapist for a 13-week contract in Durango, CO. As a Local Travel Physical Therapist, you will provide high-quality patient care in a hospital or outpatient setting while exploring all that Durango has to offer.
Key Responsibilities:
- Provide physical therapy services to patients with a variety of musculoskeletal, neurological, and post-surgical conditions.
- Develop and implement individualized treatment plans to address patient needs and goals.
- Evaluate patient progress, adjusting treatment plans as necessary to maximize functional outcomes.
- Educate patients and their families about exercises, safety techniques, and home care plans.
- Collaborate with healthcare teams, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.
- Document patient evaluations, treatments, progress, and outcomes according to clinic policies and standards.
- Assist in the discharge planning process, ensuring patients are properly transitioned from therapy services.
- Adhere to professional standards, regulations, and ethical guidelines for physical therapy practice.
Education and Certification Requirements:
- Doctor of Physical Therapy (DPT) degree from an accredited program.
- Valid Physical Therapy (PT) license in Colorado or eligibility for licensure in the state.
- Basic Life Support (BLS) certification required.
- Minimum of one (1) year of experience as a licensed Physical Therapist preferred.
- Strong knowledge of physical therapy techniques, modalities, and equipment.
Preferred Qualifications:
- Experience in outpatient, inpatient, or hospital settings.
- Ability to work independently and manage a diverse caseload.
- Strong communication and interpersonal skills for building rapport with patients and colleagues.
- Passion for working in a travel healthcare setting and exploring new communities.
