13-Week Contract | Up to $1,950/Week
Connected Health Care, a premier recruitment and staffing firm, is partnering with a respected Skilled Nursing Facility (SNF) in Alamosa, Colorado to hire an experienced Travel Physical Therapist Assistant (PTA) for a 13-week contract assignment. This is an excellent opportunity for a PTA seeking competitive weekly compensation, a consistent Monday–Friday schedule, and the chance to work in a supportive SNF environment.
Position Highlights
- Location: Alamosa, CO
- Setting: Skilled Nursing Facility (SNF)
- Contract Length: 13 Weeks
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Weekly Compensation: Up to $1,950/week (based on experience)
Job Description – Travel PTA (SNF Setting)
- Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
- Implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Assist patients recovering from orthopedic procedures, neurological conditions, and general deconditioning
- Document patient progress and communicate updates to the interdisciplinary team
- Educate patients and families on therapeutic exercises and safe mobility techniques
- Ensure compliance with facility policies, Medicare guidelines, and state regulations
- Collaborate with nursing staff and rehab team to optimize patient outcomes
- Maintain accurate and timely documentation in EMR system
Education & Certification Requirements
- Associate Degree from an accredited Physical Therapist Assistant program
- Active Colorado PTA license (or eligibility for licensure)
- Current BLS certification (American Heart Association preferred)
- Minimum 1+ year of Skilled Nursing Facility (SNF) experience required
- Strong documentation and communication skills
- Ability to work independently and as part of a collaborative rehab team
