13-Week Contract | Up to $1,950/Week


Connected Health Care, a premier recruitment and staffing firm, is partnering with a respected Skilled Nursing Facility (SNF) in Alamosa, Colorado to hire an experienced Travel Physical Therapist Assistant (PTA) for a 13-week contract assignment. This is an excellent opportunity for a PTA seeking competitive weekly compensation, a consistent Monday–Friday schedule, and the chance to work in a supportive SNF environment.
Position Highlights

  • Schedule: Monday–Friday, 8:00 AM – 5:00 PM
  • Weekly Compensation: Up to $1,950/week (based on experience)
Job Description – Travel PTA (SNF Setting)
  • Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist
  • Implement individualized treatment plans to improve mobility, strength, balance, and functional independence
  • Assist patients recovering from orthopedic procedures, neurological conditions, and general deconditioning
  • Document patient progress and communicate updates to the interdisciplinary team
  • Educate patients and families on therapeutic exercises and safe mobility techniques
  • Ensure compliance with facility policies, Medicare guidelines, and state regulations
  • Collaborate with nursing staff and rehab team to optimize patient outcomes
  • Maintain accurate and timely documentation in EMR system
Education & Certification Requirements
  • Associate Degree from an accredited Physical Therapist Assistant program
  • Active Colorado PTA license (or eligibility for licensure)
  • Current BLS certification (American Heart Association preferred)
  • Minimum 1+ year of Skilled Nursing Facility (SNF) experience required
  • Strong documentation and communication skills
  • Ability to work independently and as part of a collaborative rehab team
