Travel Social Worker – Canton, GA

Location: Canton, GA
Schedule: 3:30 PM - 12:00 AM, Monday - Friday
Contract Length: 13 Weeks

Connected Health Care is a premier travel healthcare staffing agency committed to connecting skilled professionals with top healthcare facilities across the country. We are currently seeking an experienced Acute Care Social Worker for a travel assignment in Canton, GA.

Job Responsibilities

  • Provide psychosocial support and discharge planning services to patients in a hospital-based setting
  • Assess patient needs and develop appropriate care plans
  • Collaborate with medical teams to coordinate patient care and post-discharge services
  • Offer crisis intervention and emotional support to patients and families
  • Ensure compliance with hospital policies, state regulations, and ethical guidelines

Certification & Licensing Requirements

  • Required: Georgia Licensed Clinical Social Worker (LCSW) Certification
  • Required: American Heart Association (AHA) Basic Life Support (BLS) Certification

Experience Requirements

  • Minimum of two years of experience as a Social Worker in an Acute Care Hospital setting
