Job Description

• Provide emotional support, crisis intervention, and counseling services to patients and families.
• Assess patients’ social, emotional, and financial needs to develop individualized care plans.
• Collaborate with healthcare teams to ensure comprehensive patient care and discharge planning.
• Connect patients with community resources, support groups, and financial assistance programs.
• Maintain accurate documentation of patient interactions and treatment plans.
• Advocate for patient rights and ensure access to necessary healthcare and social services.
• Adhere to ethical standards and confidentiality regulations in all interactions.

Requirements

• Education: Bachelor’s or Master’s degree in Social Work (MSW preferred).
• Certification: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Georgia required.
• Experience: Minimum of 1 year of social work experience in a healthcare setting preferred.
• Skills: Strong communication, problem-solving, and advocacy skills.
