Job Description
- Provide emotional support, crisis intervention, and counseling services to patients and families.
- Assess patients’ social, emotional, and financial needs to develop individualized care plans.
- Collaborate with healthcare teams to ensure comprehensive patient care and discharge planning.
- Connect patients with community resources, support groups, and financial assistance programs.
- Maintain accurate documentation of patient interactions and treatment plans.
- Advocate for patient rights and ensure access to necessary healthcare and social services.
- Adhere to ethical standards and confidentiality regulations in all interactions.
Requirements
- Education: Bachelor’s or Master’s degree in Social Work (MSW preferred).

- Experience: Minimum of 1 year of social work experience in a healthcare setting preferred.
