Job Requirements
  • Education Requirements: Valid Licensed Practical Nurse (LPN) license issued by the state of Wisconsin.
  • Certifications Requirements: CPR certification; Basic Life Support (BLS) preferred; IV certification (if applicable).
  • Experience Requirements: Minimum of 1 year of LPN experience in long-term care settings.

Job Responsibilities
  • Provide comprehensive bedside nursing care to residents in long-term care facilities during evening shifts.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Maintain accurate documentation of patient assessments, interventions, and outcomes in EMR systems.
  • Administer medications and treatments in accordance with facility protocols and regulations.
