Job Responsibilities

Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist

Assist patients in improving mobility, relieving pain, and achieving maximum functional performance

Accurately document daily progress notes and communicate effectively with rehab and facility staff

Follow individualized care plans tailored to each patient’s needs

Ensure compliance with facility protocols and professional standards
Requirements

Valid license as a Physical Therapist Assistant (PTA) in Oregon (or compact eligible)

Associate's degree from an accredited PTA program

Current BLS (Basic Life Support) certification

Previous SNF or geriatric experience preferred
- Strong communication and team collaboration skills
