Key Responsibilities:

Provide expert wound care services for patients in home health settings.

Assess, diagnose, and treat amniotic, diabetic, pressure, venous, and chronic wounds.

Develop and implement evidence-based treatment plans.

Collaborate with physicians and care teams to ensure optimal patient outcomes.

Maintain accurate and timely clinical documentation.

Educate patients and caregivers on wound prevention and ongoing care.
Qualifications & Requirements:

Active Nurse Practitioner (NP) license in the state of Minnesota.

Minimum of 1 year of experience in wound care, preferably in home health or clinical settings.

Demonstrated expertise in amniotic wound care and chronic wound management.

Strong communication and patient education skills.

Ability to work independently with a high degree of professionalism.
