Connected Health is seeking a compassionate and dedicated Registered Nurse (RN) to join a skilled nursing facility in Crawford, Nebraska, with a start date of May 11, 2025. Responsibilities:

• Provide direct patient care to residents in accordance with established policies and procedures.
• Monitor and assess patients' conditions, administering medications, and performing treatments as necessary.
• Develop and implement individualized care plans for residents.
• Collaborate with interdisciplinary teams to ensure optimal patient care.
• Educate patients and their families on health and wellness topics.
• Maintain accurate patient records and documentation in compliance with facility policies.

Qualifications:

• Active Registered Nurse (RN) license in the state of Nebraska.
• Previous experience in a skilled nursing facility or similar healthcare setting is preferred.
• Strong clinical and assessment skills.
• Compassionate and patient-focused with excellent communication skills.
• Ability to work independently and as part of a team.
