Job Description
As a Travel Social Worker, you will provide emotional and practical support to patients and families while collaborating with healthcare professionals to deliver the highest level of care.
Key Responsibilities:
- Assess and address patients’ psychosocial needs.
- Provide counseling and crisis intervention services.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Facilitate access to community resources, financial aid, and other support systems.
- Document all patient interactions and maintain compliance with HIPAA regulations.
- Work weekends as part of a required schedule.
Education and Certification Requirements
- Bachelor’s or Master’s Degree in Social Work (BSW/MSW) from an accredited program.
- Active and unencumbered Vermont Social Worker License (or eligibility to obtain one).
- Minimum of 1 year of experience in a healthcare or hospital setting preferred.
- Strong interpersonal, organizational, and communication skills.
