Job Description

As a Travel Social Worker, you will provide emotional and practical support to patients and families while collaborating with healthcare professionals to deliver the highest level of care.

Key Responsibilities:

  • Assess and address patients’ psychosocial needs.
  • Provide counseling and crisis intervention services.
  • Collaborate with interdisciplinary teams to develop and implement care plans.
  • Facilitate access to community resources, financial aid, and other support systems.
  • Document all patient interactions and maintain compliance with HIPAA regulations.
  • Work weekends as part of a required schedule.

Education and Certification Requirements

  • Bachelor’s or Master’s Degree in Social Work (BSW/MSW) from an accredited program.
  • Active and unencumbered Vermont Social Worker License (or eligibility to obtain one).
  • Minimum of 1 year of experience in a healthcare or hospital setting preferred.
  • Strong interpersonal, organizational, and communication skills.
